Certificate of Status Requirement (AB 1325)

Effective February 3, 2014, pursuant to Assembly Bill 1325 (enacted 1/1/14), if the Fictitious Business Name Statement registrant is a corporation, a limited liability company, or a limited liability partnership, the Los Angeles County Registrar/Recorder County Clerk's Office will require an original certificate of Status issued by the Secretary of State certifying the current existence and good standing of that business entity and shall be attached to a completed affidavit of identity statement. (A current print out from the SOS may be deemed acceptable).



Effective October 21, 2013, the Los Angeles County Registrar-Recorder/County Clerk will be implementing an enhanced automated internet application system for the filing of Fictitious Business Name Statements. The new process will allow walk-in, mail-in and third party customers (presenting Fictitious Business Name Statements on behalf of others) to utilize this web module to key, submit and print the FBN from the comfort of their own home or office. The new process will require following these instructions:

  • Use the URL web address provided by the RR/CC (https://apps.lavote.net/obfr/bfrmain.aspx)
  • Key in all required FBN fields for each document into the web module
  • Walk-in: Print out confirmation page and proceed to a designated RR/CC location along with payment for filing
  • Mail-in/Third Party: Print out the completed FBN and sign or obtain the registrant's signature on the completed FBN Statement. Mail in/deliver the FBN Statement to a designated RR/CC location along with payment for filing.

Please be sure to follow the above mentioned steps to avoid any delay in the processing of submitted FBN Statements. The new web address will be available starting on Monday, October 21, 2013.


Affidavit of Identity Form Requirement (AB 1325)

Effective January 1, 2014, pursuant to Assembly Bill 1325, the Los Angeles County Registrar/Recorder County Clerk's Office will require a completed Affidavit of Identity form to accompany the 2014 Fictitious Business Name Statement (Original, Refile and New).

a. In-Person: Registrants are required to present a completed FBN statement, show valid identification and complete the Affidavit of Identity form.

b. Mail-in: Registrants are required to submit a completed FBN statement, and notarized Affidavit of Identity form. 

c. Other: Persons presenting FBN statements on behalf of the registrant must show valid identification, and the complete Affidavit of Identify form or Authorized Agent form, where applicable.

Please note that no additional fee will be charged.

This requirement has been made in accordance with the legislative amendment of Business and Professions Code sections 17913, 17916, 17922, 17923, 17927 and 17929.


California Environmental Quality Act document fees will increase effective January 1, 2014, pursuant to Fish and Game Code §711.4(b) and 713 as follows:

  • Negative Declaration and Mitigated Negative Declaration - $2,181.25
  • Environmental Impact Report - $3029.75
  • Environmental Document prepared pursuant to a Certified Regulatory Program - $1,030.25
